United Concordia Dental coverage plans

Concordia Dental PPO networks provide easy access to reputable dentists nationwide as well as discounts on covered dental care. When you see an in-network United Concordia dentist, certain preventive care services are partially or fully covered. You pay a coinsurance (a part of the covered costs) once you meet your annual deductible and satisfy waiting periods, and the dental plan covers the remaining costs (up to the yearly or lifetime dollar limit of the plan).

Concordia Flex

Concordia Flex is a traditional dental PPO designed with flexible, affordable program options. Both its in and out-of-network percentages are the same, it boasts a large network of dentists and specialists, has ongoing credentialing, professional oversight, pre- and post-payment controls, and grievance processing to ensure the highest quality of care, and assured cost savings through pre- and post- payment utilization reviews.

Concordia Preferred

Concordia Preferred is an innovative dental PPO designed with network/non-network coverage for diagnostic, preventive, basic, major, and orthodontics services. It gives members freedom of provider choice and has significant cost savings when care is provided by network dentists. It boasts a large network of dentists and specialists, and has ongoing credentialing, professional oversight, pre- and post-payment controls, and grievance processing to ensure the highest quality of care and assured cost savings through pre- and post-payment utilization reviews.

Concordia Plus

This plan emphasizes prevention, early diagnosis, and treatment. It has stand-alone and dual-option dental programs available. Primary dentists coordinate all care including specialty referrals. It boasts a large network of dentists and specialists, and has ongoing credentialing, professional oversight, pre- and post-payment controls, and grievance processing to ensure the highest quality of care and assured cost savings through pre- and post- payment utilization reviews.

Value PPO Plan 1

A basic dental plan for younger employees and people in good oral health. Includes preventive coverage and discounts for basic and major services.

Value PPO Plan 2

A mix of preventive, basic and major dental coverage for adults and families, including our popular Smile for Health®-Wellness add-on.

Value PPO Plan 3

A low-deductible plan for cost-conscious families. Provides preventive coverage plus basic services and discounts1 for the unexpected.

Value PPO Plan 4

Broader coverage for preventive, basic and major services such as fillings, extractions and crowns. Includes our Smile for Health®-Wellness add-on.

United Concordia Dental covers Retainers?

With United Concordia Dental PPO plans, one set of post-treatment retainers (for orthodontic purposes) is typically covered for the course of a lifetime. Keep in mind that the general information given here may not apply to your benefits, limitations, maximums, waiting periods, and deductibles. For specific information about your plan’s orthodontic coverage, review your plan booklet.

United Concordia Dental Lingual Braces cost coverage?

United Concordia Dental PPO plans offer an estimated 50% coverage for lingual braces. The waiting period is 6 months while the maximum lifetime benefit is $1,500. When visiting a PPO orthodontist, you will receive the $1500 calendar year maximum.

Does United Concordia Dental cover X-Rays?

Yes, United Concordia Dental covers dental x-rays. United Concordia Dental offers full (100%) coverage of dental x-rays. United Concordia Dental plans do not require waiting periods for diagnostic services such as dental x-rays. However, some policies only pay for bitewing x‐rays, one set every 12 months, while full mouth x-rays once every 5 years.

United Concordia Dental covers Braces?

Yes, braces are covered by plans offered by United Concordia Dental. High dental plan covers up to an estimate of 70% of the cost. However, there is a maximum orthodontic coverage limit of $3,500 per person and with no waiting period. Standard dental plan also covers up to an estimate of 70% of the cost but has a $2,500 limit and 12-month waiting period.

Is Invisalign covered by United Concordia Dental?

Yes, an estimate of 70% of the total cost of your Invisalign treatment will be covered by United Concordia Dental if your plan includes orthodontic benefits. By checking the small print in your insurance policy, you can make sure that orthodontic treatments like Invisalign are covered. Watch out for a few provisions in the insurance policy, such as a non-covered expense clause, that could prevent you from receiving Invisalign coverage.

You will not be covered for Invisalign if your insurance policy specifies that orthodontics are not covered services or expenses. Additionally, keep an eye out for restrictions and exclusions. There may be a clause limiting orthodontic benefits only for those who are 19 years of age or younger. All insured members who are older than 19 will not be eligible for the orthodontic benefit, including reimbursement for Invisalign treatment, due to this specific limitation.

How much does United Concordia Dental cover for Clear Braces?

With United Concordia Dental PPO plans, clear braces are estimated to be 50% covered with a waiting period of 6 months. The maximum lifetime benefit is $1,500. You will receive the $1500 calendar year maximum when you’re visiting a PPO orthodontist.
